Dean is dead at this point. Gone and buried for 6 months, and Sam has obviously had no luck in trying to bring him back, and now hunts alone. Yet, despite the fact that Dean is not physically there, Sam still orders two meals and leaves one untouched. Emotionally, Sam seems to live as if Dean is still alive.
